The Opportunity:

We're currently looking for a Registered Nurse with Case Manager Experience to join our team. The ideal candidate will be responsible for coordinating and facilitating care for our patients, coupled with providing comprehensive nursing care when required.

Job Responsibilities:

• Provide comprehensive nursing care and perform nursing procedures
• Plan and organize care using a case management approach
• Coordinate with physicians, other health professionals, and families
• Evaluate the effectiveness of care plans and modify as necessary
• Document and communicate all care provided and outcomes

Desired Qualifications:

• Registered Nurse with a current practicing license
• Minimum 2 years experience in case management
• Experience in healthcare setting with strong clinical background
• Strong decision making and clinical judgement skills
• Excellent communication and interpersonal skills

Nice-to-Haves:

• Experience in home health care
• Specialty nursing certification
• Experience with Electronic Health Records (EHR)
• Bilingual capabilities
• Demonstrate ability to work autonomously
